Committees being assessed a civil penalty will be sent an Assessment Letter, which includes the exact
fine(s) owed by the committee, how the fines are calculated, and forms for appealing the penalty.
A committee has 30-days following the date of the Assessment Letter in which to appeal an assessment.
If the committee chooses to appeal, it should complete two of three
Civil Penalty Appeal Forms
.
The Disclosure Law also provides hearing procedures for complaints filed alleging violations of the
Law. Any person(s) that possesses substantiated information that a violation has occurred may file
a complaint, using
Form D-4. All complaint forms are filed with the Illinois State Board of Elections.
