Registration form is available at http://www.elections.il.gov/Downloads/BusinessRegistration/PDF/BEREPForm.pdf.
The registration form may be completed online and printed, or it may be printed and completed by hand. The form may be mailed or hand-delivered to the Springfield office of the State Board of Elections, 1020 South Spring Street, P.O. Box 4187, Springfield, Illinois, 62708.

The form may also be e-mailed to the Board at BEREP@elections.il.gov as an attachment in a pdf format.

Upon receipt by the Board, the front page of the Registration Form will be time stamped and returned to the business entity to serve as its Certificate of Registration. This receipting process will take place as soon as possible, but will obviously be affected by the volume of filings.

The form must be signed and dated, if not, it will be returned for signature before a certificate is issued.
